  • Overview

    Western Michigan University Homer Stryker M.D. School of Medicine is seeking a Nurse Care Coordinator.  Responsible for coordinating patient care plans, patient outreach, patient chart audits and patient compliance with orders and preventive care initiatives.  Works collaboratively with physicians and other health care workers to provide appropriate follow up and to assist with patient education.  Works collaboratively with the HIM team to provide input to clinical work flows in conjunction with EHR system upgrades and/or implementations.

     

    Responsibilities

    • Define and execute Health Maintenance Campaigns
    • Create and maintain care plans for high risk patients and other clinically relevant patients and coordinate care with primary care physicians, specialists, hospitals, community resources, social workers and other care givers.
    • Define and execute data exchange with the hospitals for admits and discharges and coordinate with the primary care physician.
    • Assist the referral specialists with patient compliance & education as needed.
    • Assist with Joint Commission, QI, HEDIS, PCMH, PQRS, Meaningful Use and other initiatives for patient follow-up, chart review and chart audit.
    • Participate in EHR upgrades/implementations to assist with validating clinical workflows and care coordination workflows.
    • Availability to work occasional overtime on weekends or evenings to complete time sensitive projects.
    • Acquires knowledge for new technology and policy/procedure revisions.
    • All other duties as assigned.

    Qualifications

    • Five years experience in a health care setting
    • Current Michigan LPN or RN license
    • Knowledge and ability to use the electronic health record (EHR)
    • Working knowledge of desktop productivity tools (Word, Excel etc.)
    • Demonstrates the ability for clear verbal and written communications
    • Knowledge of Health Promotion and Maintenance, FDA Health Regulations, Building Relationships, Resolving Conflicts, Coordination, Listening Skills, Legal and Organizational Compliance
    • Demonstrates the ability to recognize priorities in organization of work flow
    • Able to perform duties independently, with a minimal need for direct supervision

The 350,000 square foot educational building on the W.E. Upjohn M.D. Campus located in downtown Kalamazoo underwent a $78 million renovation and expansion project including two laboratory research floors and a state-of-the-art Simulation Center that is accredited by the Society for Simulation in Healthcare. The Innovation Center on the Parkview Campus is a life science, technology, and engineering incubator serving the earliest startups to maturing companies with laboratory, office and conference space, access to core scientific equipment and expertise, and a wide range of support services.
